Health questionnaire
A short pre-appointment form covering your health history, allergies and previous vaccinations. Completed before your visit.

Consultation
Brief clinical assessment to confirm suitability and discuss the vaccine. Most single-dose vaccines are administered the same visit.

Record provided
Written confirmation of your vaccination. For yellow fever, an ICVP certificate valid for life.

Common questions
- Do I need a GP referral?
- No. You can book directly with us. Our prescribing pharmacist can assess and administer privately without a GP letter.
- Are children covered?
- Eligibility varies by vaccine. Most travel vaccines are suitable from age 1 or 2; HPV from adolescence. We'll confirm eligibility at booking or during the health questionnaire.
- Can I have more than one vaccine in the same appointment?
- Often yes. We'll confirm what can be co-administered at consultation, as some combinations require timing or site separation. Book a travel consultation if you need multiple vaccines for a trip.
- What if I have allergies?
- Declare any known allergies on your health questionnaire. We observe all patients after vaccination and our pharmacist will advise on any specific allergy risk before administering.
- Do you keep a vaccination record?
- Yes. We maintain a record of all vaccinations administered. We'd recommend you also keep your own NHS vaccination record or yellow booklet updated.
If you feel unwell after a vaccination
Most side effects — soreness at the injection site, mild fatigue — are normal and resolve within 48 hours. If you experience difficulty breathing, swelling of the face or throat, or a rapid heartbeat after leaving the pharmacy, call 999 immediately. For other concerns, call NHS 24 on 111.
